The Depression Anxiety Stress Scales-21 (DASS-21) is a 21-item self-report questionnaire developed by Lovibond and Lovibond (1995) at the University of New South Wales to measure the emotional states of depression, anxiety, and stress. Each subscale contains 7 items rated on a 0–3 scale ("did not apply to me at all" to "applied to me very much, or most of the time") and sums are doubled to match norms from the original 42-item DASS (Lovibond & Lovibond, 1995, Behaviour Research and Therapy).

The DASS-21 has strong internal consistency (α = 0.82–0.94) and converges well with other validated depression and anxiety measures across clinical and community samples. Zentake's digital DASS-21 auto-scores all three subscales and applies severity bands the moment a patient submits the form.

How to Score the DASS-21

Sum the seven items of each subscale (Depression, Anxiety, Stress), then multiply by 2 to produce the final score for comparison with DASS-42 norms. Each subscale has its own severity thresholds — a score of 12 is "mild" on Depression but "moderate" on Anxiety, so always interpret subscales independently.

Zentake automatically calculates all three subscale scores, doubles them, applies the correct severity band for each, and pushes the results to the clinician dashboard upon form submission.

How to Administer the DASS-21

Step 1: Send before the visit. Deliver the DASS-21 via a HIPAA-compliant link 24–48 hours prior to the appointment. Zentake automates scheduled delivery.

Step 2: Instruct patients on the time frame. Ask the patient to rate how each statement applied to them "over the past week."

Step 3: Allow 5–10 minutes for completion. Patients complete the 21 items on any device without app installation.

Step 4: Review auto-scored results. Zentake presents Depression, Anxiety, and Stress severity bands at a glance.

Step 5: Track change over time. Use longitudinal graphs to monitor treatment response and flag deterioration. Zentake visualizes DASS-21 trends for every patient.

Who Uses the DASS-21?

Digital vs. Paper DASS-21

Scoring: Paper requires manually summing three subscales and doubling each; Zentake does it instantly with correct severity banding.

Completion: Paper adds waiting-room overhead; Zentake allows pre-visit completion.

Delivery: Paper must be printed; Zentake sends HIPAA-compliant links by SMS or email.

Data Storage: Paper fills file cabinets; Zentake stores results in an encrypted database.

Longitudinal Tracking: Paper makes trending tedious; Zentake graphs change automatically.

Security: Paper is vulnerable to loss; Zentake is HIPAA-compliant and encrypted end-to-end.

Integration: Paper requires manual EMR entry; Zentake syncs DASS-21 results directly.

Cost: Paper consumes staff time; Zentake eliminates the administrative burden.

Frequently Asked Questions

What does the DASS-21 measure?

The DASS-21 measures three negative emotional states: depression (dysphoria, hopelessness, anhedonia), anxiety (autonomic arousal, situational anxiety), and stress (tension, agitation, irritability). Zentake delivers and auto-scores the DASS-21 digitally.

How do you score the DASS-21?

Sum the seven items of each subscale and multiply by 2. Then compare each subscale score to its own severity cutoffs. Zentake calculates and bands all three subscales automatically.

How long does the DASS-21 take?

The DASS-21 takes 5–10 minutes to complete. Zentake's mobile-optimized form allows patients to complete it before the visit.

Is the DASS-21 free to use?

Yes — the DASS-21 is in the public domain and free for clinical, research, and educational use. Zentake provides a ready-to-use digital version.

What age or population is the DASS-21 for?

The DASS-21 is validated for adults aged 17 and up. The DASS-Y is available for children and adolescents.

Who should administer the DASS-21?

The DASS-21 is self-administered. Clinicians interpret the scores. Zentake fully automates delivery, scoring, and reporting.

Why is the DASS-21 score multiplied by two?

Doubling allows comparison with the norms from the original 42-item DASS. Zentake handles this calculation automatically.
